Leponex (Clozapine)

Leponex (clozapine) is used within the antipsychotic therapeutic class to manage severe psychotic disorders that have not responded to standard treatments.

Common Side Effects

• Sedation: drowsiness or fatigue.
• Weight gain: increased appetite leading to weight increase.
• Hypersalivation: excessive saliva production.
• Constipation: reduced bowel motility.
• Dizziness: light‑headedness, especially on standing.
• Tachycardia: elevated heart rate.

Serious or Rare Side Effects

• Agranulocytosis: severe reduction in white blood cells, requiring regular blood monitoring.
• Myocarditis: inflammation of the heart muscle, presenting with chest pain or shortness of breath.
• Seizures: risk increases with higher doses or pre‑existing seizure disorders.
• Orthostatic hypotension: marked drop in blood pressure upon standing.
• Neuroleptic malignant syndrome: life‑threatening reaction with fever, rigidity, and autonomic instability.

Clozapine therapy requires careful monitoring and patient education to minimise risks.

• Blood count monitoring: perform baseline and regular absolute neutrophil count tests.
• Cardiac assessment: evaluate for myocarditis signs, especially during early treatment.
• Seizure risk: avoid abrupt dose changes and assess seizure history.
• Metabolic monitoring: track weight, glucose and lipid levels.
• Drug discontinuation: taper gradually under medical supervision to prevent withdrawal effects.

• Carbamazepine: induces metabolism, markedly lowering clozapine levels.
• Fluvoxamine: strong CYP1A2 inhibitor, causing potentially toxic clozapine accumulation.
• Smoking cessation: loss of CYP1A2 induction may raise clozapine concentrations.
