Clozapine is an atypical antipsychotic used in the management of psychotic disorders that are resistant to standard therapy.

• Clozapine exerts its antipsychotic effect primarily by antagonising dopamine D2 receptors and serotonin 5‑HT2A receptors, with additional affinity for dopamine D4, α1‑adrenergic, muscarinic and histamine H1 receptors. This broad receptor profile modulates dopaminergic and serotonergic neurotransmission, leading to reduced positive and negative psychotic symptoms while minimizing extrapyramidal side effects. It also influences glutamatergic transmission and may increase cortical GABA activity, contributing to its efficacy in refractory cases.

Clozapine may cause a range of adverse reactions, varying from mild to potentially life‑threatening.

Common Side Effects

• Sedation: drowsiness or sleepiness, especially at treatment initiation.
• Weight gain: increase in body weight and appetite.
• Constipation: reduced bowel movements, may require dietary measures.
• Salivation: excessive drooling or hypersalivation.
• Dizziness: light‑headedness, particularly when standing quickly.

• Metabolic syndrome: significant changes in glucose and lipid metabolism.
