Lercanidipin Amarox (Lercanidipine Hydrochloride Hemihydrate)

Lercanidipine is a dihydropyridine calcium channel blocker indicated for the management of hypertension.

• Essential hypertension: lowers systolic and diastolic blood pressure in patients with primary hypertension.
• Isolated systolic hypertension: provides vascular relaxation to reduce elevated systolic pressure.
• Secondary hypertension (when appropriate): assists in achieving blood pressure control alongside other antihypertensive agents.

• Lercanidipine selectively blocks L‑type calcium channels in vascular smooth muscle, inhibiting calcium influx. This leads to relaxation of arterial smooth muscle, vasodilation, and a consequent reduction in peripheral vascular resistance, thereby lowering systemic blood pressure.

Lercanidipine may cause a range of adverse reactions.

Common Side Effects

• Headache: mild to moderate intensity.
• Flushing: transient warmth and redness of the skin.
• Peripheral edema: swelling of the ankles or lower legs.
• Dizziness: sensation of light‑headedness, especially on standing.

Serious or Rare Side Effects

• Angioedema: swelling of the face, lips, tongue, or throat requiring emergency care.
• Severe hypotension: marked drop in blood pressure leading to fainting.
• Myocardial infarction: rare occurrence of heart attack.
• Hepatic dysfunction: abnormal liver enzyme elevations or hepatitis.

Before initiating therapy, consider the following safety measures.

• Pregnancy: safety not established; avoid unless clearly needed.
• Renal impairment: monitor blood pressure and adjust therapy if required.
• Hepatic impairment: use with caution and observe for increased exposure.
• Concomitant CYP3A4 inhibitors: may raise plasma levels, increasing risk of adverse effects.
• Abrupt discontinuation: may cause rebound hypertension; taper when stopping.
• Store at room temperature: keep in the original pack, protect from moisture and light.

Major Interactions (Avoid)

• CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin): increase lercanidipine concentrations.
