Leuject (Cladribine)

Leuject (cladribine) is indicated for the treatment of a specific B‑cell malignancy.

• Hairy cell leukemia: induces remission by depleting malignant B‑lymphocytes and restoring normal hematopoiesis.

• Cladribine is a nucleoside analog that is taken up by lymphocytes and phosphorylated intracellularly. The phosphorylated form incorporates into DNA during replication, causing strand breaks and triggering apoptosis of proliferating lymphocytes, particularly malignant B‑cells. This selective cytotoxicity underlies its antineoplastic activity in hairy cell leukemia.

Cladribine may cause a range of adverse reactions, most of which are related to its immunosuppressive and cytotoxic properties.

Common Side Effects

• Injection site pain or erythema: mild discomfort at the subcutaneous site.
• Fatigue: transient tiredness during treatment cycles.
• Nausea: occasional upset stomach, often manageable with anti‑emetics.
• Lymphopenia: reduced lymphocyte counts, monitored with blood tests.
• Headache: mild to moderate headache episodes.
• Fever: low‑grade fever may occur post‑injection.

Serious or Rare Side Effects

• Severe infections: opportunistic bacterial, viral or fungal infections requiring prompt treatment.
• Autoimmune hemolytic anemia: destruction of red blood cells leading to anemia.
• Secondary malignancies: rare development of new cancers after prolonged exposure.
• Severe neutropenia: marked drop in neutrophils increasing infection risk.
• Progressive multifocal leukoencephalopathy: rare viral brain infection.
• Hypersensitivity reactions: rash, bronchospasm or anaphylaxis in susceptible individuals.

Prior to initiating therapy, clinicians should assess patient suitability and monitor for potential complications.

• Blood count monitoring: perform complete blood counts regularly to detect cytopenias.
• Infection surveillance: evaluate for active infections and advise prophylactic measures when indicated.
• Renal function assessment: check kidney function as impaired clearance may affect drug exposure.
• Pregnancy avoidance: women of childbearing potential should use effective contraception; pregnancy category is unknown.
• Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake the vial.

Major Interactions (Avoid)

• Concurrent immunosuppressants: may cause profound immunosuppression and increase infection risk.
• Live vaccines: administration can lead to uncontrolled infection due to impaired immunity.
