Levobupivacaina Molteni (Levobupivacaine)

Levobupivacaina is a local anesthetic used in epidural techniques to manage pain and facilitate surgery.

• Epidural anesthesia for surgery: produces reversible loss of sensation in the operative region, allowing procedures without general anesthesia.
• Labor and delivery analgesia: provides effective pain relief while preserving maternal consciousness and motor function.
• Post‑operative pain management: extends analgesia after surgery, reducing the need for systemic opioids.

• Levobupivacaine reversibly binds to the intracellular portion of voltage‑gated sodium channels in neuronal membranes, stabilising the inactive state of the channel. This blocks the influx of sodium ions required for depolarisation, preventing the initiation and propagation of nerve impulses and resulting in a reversible loss of sensory and motor function in the targeted area.

Adverse reactions may occur following epidural administration of levobupivacaine.

Common Side Effects

• Hypotension: decreased blood pressure due to sympathetic blockade.
• Bradycardia: slowed heart rate, especially with high spinal spread.
• Nausea and vomiting: gastrointestinal upset during onset.
• Dizziness or light‑headedness: related to blood pressure changes.
• Pruritus: itching at the injection site or generalized.

Serious or Rare Side Effects

• Cardiac arrest: severe arrhythmia or conduction block.
• Seizures: central nervous system toxicity from high plasma levels.
• Respiratory depression: inadequate ventilation, particularly with high blocks.
• Allergic reaction: rash, urticaria, or anaphylaxis.
• Spinal hematoma: bleeding in the epidural space causing neurologic compromise.
• Persistent neurologic deficit: prolonged sensory or motor loss beyond expected duration.

When using levobupivacaina, clinicians should observe specific safety measures.

• Dose adjustment in elderly: monitor hemodynamics and consider reduced volume.
• Avoid in severe cardiac disease: risk of arrhythmias and conduction disturbances.
• Use caution with concomitant CNS depressants: may enhance sedation or respiratory depression.
• Assess coagulation status before epidural placement: to minimise risk of epidural hematoma.
• Store at ambient temperature: keep in original container, protect from light and moisture.

Levobupivacaina may interact with other agents that affect cardiac or central nervous system function.

Major Interactions (Avoid)

• Class I antiarrhythmics (e.g., quinidine): increase risk of cardiotoxicity.
• Beta‑blockers: potentiate bradycardia and hypotension.
• High‑dose general anesthetics (e.g., propofol): additive central nervous system depression.

Moderate Interactions (Monitor Closely)

• Other local anesthetics (e.g., lidocaine): cumulative systemic toxicity.
• Opioids: enhanced respiratory depression when combined epidurally.
• Magnesium sulfate: may potentiate neuromuscular blockade and affect cardiac conduction.
