Levobupivacaine (Levobupivacaine Hydrochloride)

Levobupivacaine is employed to achieve analgesia and anesthesia in a range of surgical and obstetric settings.

• Levobupivacaine exerts its effect by reversibly binding to voltage‑gated sodium channels on neuronal membranes, stabilising the inactive state of the channel. This blocks the influx of sodium ions required for depolarisation, thereby preventing initiation and propagation of nerve impulses and producing a reversible loss of sensation in the targeted area.

Adverse reactions may occur following administration of levobupivacaine.

Common Side Effects

• Hypotension: drop in blood pressure during regional administration.
• Bradycardia: slower than normal heart rate observed after epidural use.
• Dizziness or light‑headedness.
• Nausea or vomiting.
• Metallic taste in the mouth.
• Headache.

Serious or Rare Side Effects

• Cardiac arrhythmia: potentially life‑threatening rhythm disturbances.
• Seizures: central nervous system excitation at high plasma concentrations.
• Respiratory depression: reduced breathing effort, especially with intrathecal dosing.
• Prolonged motor block: extended loss of muscle function.
• Allergic reaction: rash, urticaria or anaphylaxis.
• Methemoglobinemia: rare alteration of hemoglobin oxygen‑carrying capacity.

Careful assessment and monitoring are required when using levobupivacaine.

• Monitor cardiovascular status: observe for hypotension and bradycardia during administration.
• Assess neurological function: watch for signs of excessive motor block or sensory loss.
• Use incremental dosing: avoid large bolus injections to reduce toxicity risk.
• Evaluate patient history: consider pre‑existing cardiac or hepatic disease.
• Avoid intravascular injection: aspirate before each dose.
• Store at ambient temperature: keep in original container, protect from light and moisture.

Major Interactions (Avoid)
