Levolac (Lactulose, Liquid)

Levolac (lactulose) is used in the management of conditions related to bowel motility and ammonia metabolism.

• Constipation: softens stool and promotes regular bowel movements in adults and children.
• Hepatic encephalopathy: reduces colonic ammonia production, helping improve mental status in patients with liver dysfunction.
• Prevention of hepatic encephalopathy recurrence: maintains low ammonia levels to reduce the risk of repeat episodes in chronic liver disease.

• After oral administration, lactulose reaches the colon where bacterial enzymes hydrolyze it to short‑chain fatty acids such as lactic and acetic acid. This creates an osmotic gradient that draws water into the lumen, softening stool and stimulating peristalsis. The resulting acidic environment converts ammonia (NH3) to ammonium (NH4+), which is poorly absorbed, thereby lowering systemic ammonia concentrations.

Lactulose may cause a range of adverse reactions, most of which are related to its osmotic activity in the gastrointestinal tract.

Common Side Effects

• Flatulence: increased gas from bacterial fermentation.
• Diarrhea: loose stools due to excess water in the colon.
• Abdominal cramping: discomfort from heightened intestinal motility.

Serious or Rare Side Effects

• Electrolyte imbalance: dehydration that may cause low potassium or sodium.
• Severe abdominal pain: could indicate intestinal obstruction.
• Allergic reaction: rash, itching or swelling.

When prescribing lactulose, several safety considerations should be observed to minimize risks and ensure appropriate use.

• Assess renal function: adjust use in patients with severe renal impairment.
• Monitor fluid and electrolyte status: especially in elderly or those on diuretics.
• Evaluate hepatic status: use under supervision in liver disease patients.
• Avoid in intestinal obstruction: contraindicated when blockage is present.
• Use caution in diabetic patients: consider carbohydrate content of the solution.
• Store at ambient temperature: keep below 25 °C, retain in original container and protect from moisture.

Lactulose can interact with other medicines, affecting their absorption or therapeutic effect.

Major Interactions (Avoid)

• Antacids containing magnesium or aluminum: may reduce lactulose efficacy by altering gut pH.
• Oral antibiotics: can diminish bacterial fermentation of lactulose, lowering its osmotic effect.

Moderate Interactions (Monitor Closely)

• Diuretics: increased risk of electrolyte disturbances due to fluid loss.
• Insulin or oral hypoglycemics: carbohydrate content may affect blood glucose control.
• Other laxatives: additive effect may cause severe diarrhea.
