• Levosimendan is used in the management of heart failure, acting as a calcium sensitizer and vasodilator.
• Acute decompensated heart failure: improves cardiac output and relieves symptoms of congestion.
• Severe chronic heart failure: provides hemodynamic support when standard therapy is insufficient.
• Heart failure with reduced ejection fraction (HFrEF): enhances myocardial contractility without increasing intracellular calcium.

• Levosimendan binds to troponin C in cardiac myocytes, increasing its sensitivity to calcium and thereby augmenting contractile force without raising intracellular calcium levels. It also opens ATP‑sensitive potassium channels in vascular smooth muscle, causing vasodilation of both arterial and venous beds, which reduces preload and afterload and improves overall cardiac performance.

Levosimendan may cause a range of adverse reactions, from mild to serious.

Common Side Effects

• Hypotension: reduction in blood pressure, especially after rapid infusion.
