• Levosimendan increases myocardial contractility by binding to troponin C and stabilizing its calcium‑bound state, which sensitizes the contractile apparatus to existing calcium. Simultaneously, it opens ATP‑sensitive potassium channels in vascular smooth muscle, causing vasodilation and reducing after‑load, thereby improving overall cardiac efficiency without raising intracellular calcium concentrations.

Levosimendan is generally well tolerated, but several adverse reactions have been reported.

Common Side Effects

• Headache: mild to moderate intensity, often transient.
• Nausea or vomiting: may occur during or shortly after infusion.
• Hypotension: dose‑related drop in blood pressure.
• Dizziness or light‑headedness: related to vasodilatory effect.
• Flushing: warm sensation and reddening of skin.
• Arrhythmias: occasional premature beats.

Serious or Rare Side Effects

• Severe hypotension: may require vasopressor support.
• Ventricular tachyarrhythmias: potentially life‑threatening.
• Myocardial ischemia: chest pain or ECG changes.
• Anaphylactic reactions: rare but require immediate intervention.
