Levosimendan, an inodilator, is employed in the treatment of severe cardiac failure conditions.

• Levosimendan binds to cardiac troponin C, increasing its sensitivity to calcium and thereby augmenting contractile force without raising intracellular calcium concentrations. Simultaneously, it opens ATP‑sensitive potassium channels in vascular smooth muscle, causing arterial and venous dilation, which reduces preload and afterload. These combined actions improve cardiac output in patients with compromised heart function, especially during acute decompensation and in cardiogenic shock.
