Lexacin (Levofloxacin)

Lexacin (levofloxacin) is used to treat bacterial infections of the eye by targeting common ocular pathogens.

• Bacterial conjunctivitis: eliminates causative bacteria, relieving redness, discharge, and irritation.
• Bacterial keratitis: eradicates infiltrating organisms, promoting corneal healing and preserving vision.
• Other ocular bacterial infections: controls infection in conditions such as blepharitis or postoperative endophthalmitis, reducing microbial load.

• Levofloxacin inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA supercoiling and replication. By binding to the ATP‑binding sites of these enzymes, the drug prevents DNA unwinding and transcription, leading to bacterial cell death. This bactericidal activity is broad‑spectrum against Gram‑negative and Gram‑positive ocular pathogens, making it effective for topical ophthalmic applications.

Levofloxacin ophthalmic solution may cause local and systemic adverse reactions, most of which are mild and transient.

Common Side Effects

• Eye irritation: transient burning or stinging upon instillation.
• Redness: mild conjunctival hyperemia.
• Foreign body sensation: feeling of grit in the eye.
• Blurred vision: temporary visual disturbance after dosing.

Serious or Rare Side Effects

• Tendon rupture: rare systemic tendon injury, especially in patients with risk factors.
• Severe allergic reaction: anaphylaxis or angioedema requiring immediate medical attention.
• Corneal ulceration: worsening of corneal integrity in susceptible individuals.

Before prescribing Lexacin, clinicians should consider the following precautions to ensure safe use.

• Patient history: assess for hypersensitivity to fluoroquinolones or other antibiotics.
• Systemic exposure: avoid use in patients with a history of tendon disorders or recent corticosteroid therapy.

• Contact lens wearers: advise removal of lenses before instillation and delay reinsertion to reduce drug interaction.
