Lidagliptin Stada (Linagliptin)

Linagliptin is a DPP‑4 inhibitor used in the treatment of type 2 diabetes mellitus.

• Type 2 diabetes mellitus (monotherapy): improves glycaemic control as a standalone treatment.
• Type 2 diabetes mellitus (in combination with metformin): enhances glucose lowering when added to metformin.
• Type 2 diabetes mellitus (in combination with insulin): provides additional HbA1c reduction when used with insulin therapy.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, which degrades incretin hormones such as GLP‑1 and GIP. By blocking DPP‑4, linagliptin increases circulating incretin levels, leading to glucose‑dependent enhancement of insulin secretion from pancreatic β‑cells and suppression of glucagon release from α‑cells, thereby lowering blood glucose concentrations.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nasal passages and throat.
• Headache: transient mild to moderate head pain.
• Upper respiratory tract infection: symptoms of cough, sore throat, or congestion.
• Dizziness: occasional light‑headed sensation.
• Gastrointestinal upset: nausea, abdominal discomfort, or diarrhoea.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe hypersensitivity reactions: rash, angioedema, or anaphylaxis.
• Hepatic injury: elevated liver enzymes or jaundice.
• Bullous pemphigoid: rare blistering skin disorder.
• Acute renal failure: sudden loss of kidney function in predisposed patients.

Before initiating therapy, clinicians should consider several safety aspects.

• Renal impairment: monitor renal function as linagliptin is excreted unchanged.
• Hepatic disease: use with caution in patients with liver dysfunction.
• Hypersensitivity: discontinue if signs of allergic reaction appear.
• Pancreatitis: advise patients to report persistent abdominal pain.
• Pregnancy and lactation: safety not established; weigh benefits against potential risks.
• Store at room temperature: keep in the original container, protect from moisture and light.

Linagliptin may interact with other medicines, requiring attention to avoid reduced efficacy or increased toxicity.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., ketoconazole): may increase linagliptin exposure.
• Strong CYP3A4 inducers (e.g., rifampin): may reduce linagliptin exposure.
• Insulin secretagogues (e.g., sulfonylureas): increased risk of hypoglycaemia.

Moderate Interactions (Monitor Closely)

• Diuretics: may affect glucose control; monitor blood glucose.
• ACE inhibitors: observe renal function and electrolytes during combined therapy.
