Lidoposterin (Lidocaine)

Lidoposterin (lidocaine) is used in the management of pain and inflammation within the anorectal region.

• Anal fissure: reduces pain and facilitates healing by numbing the affected tissue.
• Hemorrhoidal disease: provides temporary relief of discomfort and itching associated with internal or external hemorrhoids.
• Proctitis: alleviates inflammatory pain in the rectal mucosa during acute episodes.

• Lidocaine reversibly blocks voltage‑gated sodium channels on neuronal membranes, preventing the initiation and propagation of nerve impulses. By stabilising the neuronal membrane, it inhibits the transmission of pain signals from the rectal mucosa, producing a localized anesthetic effect without significant systemic activity when used as directed.

Lidoposterin may cause a range of adverse reactions, most of which are related to its local anesthetic properties.

Common Side Effects

• Local burning or stinging at the application site.
• Numbness or tingling of the perianal skin.
• Mild constipation.
• Headache.
• Nausea.
• Transient dizziness.

Serious or Rare Side Effects

• Allergic reactions such as rash, urticaria, or angioedema.
• Methemoglobinemia leading to cyanosis.
• Seizures due to systemic absorption.
• Cardiac arrhythmias including ventricular tachycardia.
• Severe hypotension or cardiovascular collapse.

When using Lidoposterin, several safety considerations should be observed to minimize risk.

• Allergy assessment: verify no known hypersensitivity to lidocaine or other amide anesthetics.
• Dose limitation: employ the lowest effective amount for the shortest duration.
• Systemic toxicity monitoring: watch for signs of CNS or cardiovascular effects.
• Cardiac disease caution: avoid in patients with severe conduction abnormalities.
• Hepatic impairment: use with caution in patients with significant liver dysfunction.
• Store at room temperature: keep in the original container, protect from moisture and direct sunlight.

Lidoposterin can interact with other medicines that affect sodium channels or hepatic metabolism.

Major Interactions (Avoid)

• Class I antiarrhythmics (e.g., quinidine, procainamide): may potentiate cardiac toxicity.
• Amiodarone: increases lidocaine plasma levels and risk of arrhythmias.
• Cimetidine: reduces hepatic clearance, raising systemic exposure.
• Beta‑blockers (e.g., propranolol): can enhance bradycardia and hypotension.
• Other local anesthetics: additive neuro‑ and cardiotoxic effects.
