Ligosan (Doxycycline)

Doxycycline is used in the management of bacterial infections affecting the periodontal tissues.

• Periodontal disease: reduces bacterial load and inflammation, supporting tissue stability.
• Chronic periodontitis: helps to halt progression of attachment loss and pocket depth.
• Aggressive periodontitis: contributes to microbial control in rapidly progressing cases.

• Doxycycline penetrates gingival crevicular fluid and binds to the 30S ribosomal subunit of susceptible bacteria, blocking the addition of aminoacyl‑tRNA to the mRNA‑ribosome complex. This inhibition of protein synthesis is bacteriostatic, limiting bacterial growth. At sub‑antimicrobial doses it also inhibits matrix metalloproteinases, modulating the host inflammatory response in periodontal tissues.

Doxycycline may cause a range of adverse reactions, varying from mild to severe.

Common Side Effects

• Nausea: mild stomach upset.
• Diarrhea: loose stools.
• Photosensitivity: increased skin sensitivity to sunlight.
• Taste alteration: metallic or bitter taste.
• Esophageal irritation: discomfort when swallowing.

Serious or Rare Side Effects

• Severe allergic reaction: rash, swelling, anaphylaxis.
• Hepatotoxicity: signs of liver injury such as jaundice.
• Pseudomembranous colitis: severe diarrhea with abdominal pain.
• Blood dyscrasias: low white blood cells or platelets.
• Intracranial hypertension: headache, visual disturbances.

When prescribing Ligosan, several safety considerations should be observed.

• Pregnancy: contraindicated; avoid use during pregnancy due to risk of fetal tooth discoloration and growth restriction.
• Breastfeeding: not recommended; doxycycline can be excreted in milk and affect the infant.
• Age restriction: not indicated for children under 8 years because of permanent tooth staining.
• Renal impairment: dose adjustment may be required; monitor kidney function.
• Drug‑induced photosensitivity: advise patients to limit sun exposure and use protective clothing.
• Store at ambient temperature: keep below 25 °C, in the original container, protected from moisture and light.

Doxycycline interacts with several medicines; awareness of the interaction level guides clinical decisions.

Major Interactions (Avoid)

• Antacids containing aluminum, calcium, or magnesium: reduce doxycycline absorption.
• Iron supplements: chelate doxycycline and decrease its bioavailability.
• Warfarin: may enhance anticoagulant effect, increasing bleeding risk.
• Oral contraceptives: may reduce efficacy, increasing risk of unintended pregnancy.

Moderate Interactions (Monitor Closely)

• Retinoids (e.g., isotretinoin): increased risk of intracranial hypertension.
• Penicillins: antagonistic effect may reduce antibacterial activity.
• Statins: potential for increased muscle toxicity.
