Ligynax (Clotrimazole)

Clotrimazole is used to treat fungal infections of the vaginal mucosa, primarily those caused by Candida species.

• Vaginal candidiasis: effective relief of itching, burning and discharge associated with acute infection.
• Recurrent vaginal candidiasis: helps prevent recurrence when used as prescribed for maintenance therapy.
• Yeast infection: provides topical antifungal action to eradicate Candida overgrowth in the vaginal environment.

• Clotrimazole exerts its antifungal activity by inhibiting the fungal enzyme lanosterol 14α‑demethylase, a key component of the ergosterol biosynthesis pathway. Blocking this enzyme prevents the conversion of lanosterol to ergosterol, disrupting cell membrane formation and increasing membrane permeability, which leads to fungal cell death. The inhibition occurs locally at the site of application, allowing high drug levels in vaginal tissue while minimizing systemic exposure. By depleting ergosterol, the drug compromises membrane integrity and interferes with fungal metabolic processes.

Clotrimazole may cause local or systemic adverse reactions, most of which are mild and reversible.

Common Side Effects

• Local irritation: mild burning, stinging or redness at the site of application.
• Pruritus: increased itching of the vaginal area during or after treatment.
• Vaginal discharge: watery or white discharge that is not due to infection.
• Dryness: temporary vaginal dryness or a feeling of tightness.
• Vaginal odor: a transient change in vaginal odor that resolves after therapy.

Serious or Rare Side Effects

• Allergic reaction: rash, hives, swelling of face or lips, or difficulty breathing indicating hypersensitivity.
• Severe irritation: intense pain, ulceration, or persistent inflammation that does not improve with discontinuation.
• Systemic effects: rare reports of liver enzyme elevation or blood abnormalities with extensive or prolonged use.

When using Ligynax, consider the following precautions to ensure safe and effective therapy.

• Pregnancy: safety has not been established; consult a healthcare professional before use.
• Breastfeeding: unknown effect on infant; discuss risks with a physician.
• Allergies: avoid if known hypersensitivity to clotrimazole or other imidazoles.
• Concomitant vaginal products: do not use other medicated creams, douches, or spermicides during treatment.
• Prolonged use: limit duration to the prescribed period to reduce risk of resistance.
• Store at room temperature: keep below 25 °C, in the original packaging, protected from moisture and direct sunlight.

Clotrimazole can interact with certain medicines and substances; be aware of the following.

Major Interactions (Avoid)

• Systemic azoles: concurrent use of oral ketoconazole or itraconazole may increase systemic exposure and toxicity.
• CYP450 inhibitors: strong inhibitors such as ketoconazole can raise clotrimazole levels when absorbed systemically.
• Alcohol-based products: may enhance irritation when used with alcohol‑containing vaginal preparations.

Moderate Interactions (Monitor Closely)

• Hormonal contraceptives: topical clotrimazole may reduce efficacy of estrogen‑based methods; consider backup contraception.
• Antifungal resistance: prolonged use with other antifungals can select for resistant Candida strains.
• Vaginal lubricants: some lubricants may alter drug absorption; monitor therapeutic response.
