Indications & Clinical Uses

Amikacin is used to treat infections caused by susceptible Gram‑negative bacteria.

  • Severe urinary tract infections: eradicates susceptible Gram‑negative pathogens causing complicated UTIs.
  • Hospital‑acquired pneumonia: provides bactericidal activity against resistant Gram‑negative organisms in the lungs.
  • Complicated intra‑abdominal infections: targets Gram‑negative bacteria involved in peritonitis and abscesses.
  • Septicemia (bloodstream infection): delivers rapid bacterial kill in systemic Gram‑negative infections.
  • Febrile neutropenia: used as part of combination therapy for Gram‑negative sepsis in neutropenic patients.
  • Bone and joint infections: administered for osteomyelitis caused by susceptible Gram‑negative organisms.

How It Works

  • Amikacin binds irreversibly to the bacterial 30S ribosomal subunit, causing misreading of mRNA and inhibition of protein synthesis. This action leads to rapid bactericidal activity against susceptible Gram‑negative organisms. The drug enters bacterial cells via an oxygen‑dependent transport system, and its activity is concentration‑dependent, making high peak concentrations essential for optimal effect. It is ineffective against anaerobes and Gram‑positive cocci, and resistance may develop through enzymatic modification or target mutation.

Side Effects

Amikacin may cause a range of adverse reactions, varying from mild to severe.

Common Side Effects

  • Nephrotoxicity: elevated serum creatinine and reduced glomerular filtration.
  • Ototoxicity: high‑frequency hearing loss or tinnitus.
  • Neuromuscular blockade: muscle weakness or respiratory depression.
  • Injection site reactions: pain, erythema, or swelling at the administration site.

Serious or Rare Side Effects

  • Acute renal failure: sudden loss of kidney function requiring dialysis.
  • Permanent hearing loss: irreversible auditory damage.
  • Anaphylaxis: severe allergic reaction with hypotension and airway compromise.
  • Balance disturbances: vertigo or loss of equilibrium.

Precautions & Warnings

Before initiating amikacin therapy, clinicians should consider several precautionary measures.

  • Renal function monitoring: assess serum creatinine and urine output before and during treatment.
  • Audiometric testing: obtain baseline hearing assessment and repeat periodically.
  • Pregnancy risk: use only when benefits outweigh potential fetal harm.
  • Concomitant nephrotoxic drugs: avoid or adjust dose when given with other renal toxins.
  • Dosage adjustment in the elderly: consider reduced clearance and monitor levels.
  • Store at ambient temperature: keep in original container, protect from moisture and light.

Avoid Interactions

Amikacin can interact with other medicines, influencing its safety and effectiveness.

Major Interactions (Avoid)

  • Co‑administration with other aminoglycosides: markedly increased risk of nephro‑ and ototoxicity.
  • Loop diuretics (e.g., furosemide): synergistic renal toxicity.
  • Neuromuscular blocking agents (e.g., vecuronium): enhanced muscle paralysis.
