Linagliptin is indicated for the management of type 2 diabetes mellitus, a chronic metabolic disorder characterized by insulin resistance.

• Type 2 diabetes mellitus (monotherapy): lowers HbA1c and fasting glucose when used alone.
• Type 2 diabetes mellitus (combination therapy): used with metformin, sulfonylureas or insulin to achieve additional glycemic control.
• Patients with renal impairment: can be administered without dose adjustment, providing a convenient option for those with reduced kidney function.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, which degrades the incretin hormones glucagon‑like peptide‑1 (GLP‑1) and glucose‑dependent insulinotropic polypeptide (GIP). By blocking DPP‑4, linagliptin raises circulating GLP‑1 and GIP levels, enhancing glucose‑dependent insulin secretion, suppressing glucagon release, and improving both post‑prandial and fasting glycaemia without causing hypoglycaemia when used alone.

Linagliptin may be associated with several adverse reactions, which are categorized by frequency and severity.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nose and throat.
• Headache: transient cranial discomfort.
• Gastrointestinal upset: nausea, abdominal pain or diarrhoea.
• Upper respiratory tract infection: cough and congestion.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with elevated enzymes.
• Severe allergic reactions: rash, angioedema or anaphylaxis.
• Hepatic injury: jaundice or elevated liver enzymes.

When prescribing linagliptin, clinicians should consider several precautionary measures to ensure safe use.

• Renal function: monitor kidney status, although dose adjustment is not required.
• Hepatic impairment: use with caution in patients with liver disease.
• Hypersensitivity: discontinue if signs of allergic reaction appear.
• Pregnancy: avoid unless the potential benefit justifies the risk, as safety data are limited.
• Concomitant antidiabetic agents: adjust doses of insulin or sulfonylureas to reduce hypoglycaemia risk.

• Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole): may increase linagliptin exposure.
