Linagliptin is a DPP‑4 inhibitor used in the management of type 2 diabetes mellitus, either alone or with other antidiabetic agents.

• Type 2 diabetes mellitus (monotherapy): improves glycaemic control by enhancing insulin secretion and reducing glucagon release.
• Type 2 diabetes mellitus (combination therapy): used together with metformin, sulfonylureas or insulin to achieve target HbA1c levels.
• Type 2 diabetes mellitus (renal impairment): provides effective glucose lowering without dose adjustment in mild to moderate renal dysfunction.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the degradation of endogenous incretin hormones such as GLP‑1 and GIP. Elevated incretin levels increase glucose‑dependent insulin secretion from pancreatic β‑cells and suppress glucagon release from α‑cells, resulting in reduced hepatic glucose production and improved post‑prandial glucose control.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection symptoms.
• Headache: transient mild to moderate headache.
• Dizziness: occasional light‑headedness.
• Gastrointestinal discomfort: nausea, abdominal pain or diarrhoea.
• Back pain: non‑specific musculoskeletal pain.
• Skin rash: mild erythematous rash.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with elevated enzymes.
• Severe allergic reactions: angio‑edema, urticaria, anaphylaxis.
• Hepatic injury: jaundice, elevated liver enzymes.
• Hypoglycaemia (when combined with insulin or sulfonylureas): symptomatic low blood glucose.
• Bullous pemphigoid: rare autoimmune skin blistering disorder.

Linagliptin should be used with clinical vigilance in the following situations:

• Renal impairment: monitor renal function; dose adjustment may be required in severe cases.
• Hepatic impairment: assess liver enzymes regularly.
• Cardiovascular disease: use with caution in patients with unstable heart conditions.
• Pregnancy: safety not established; prescribe only if potential benefit justifies risk.
• Elderly patients: monitor for increased adverse events due to polypharmacy.
• Store at ambient temperature: keep in original packaging, protect from moisture and direct sunlight.

Linagliptin may interact with several medicines; awareness of the interaction level helps guide monitoring.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole): may increase linagliptin exposure.
