• Linagliptin is used in the management of type 2 diabetes mellitus, acting on the incretin pathway to improve glucose regulation.
• Type 2 diabetes mellitus: improves overall glycaemic control as monotherapy or in combination with other antidiabetic agents.
• Adults with type 2 diabetes: reduces HbA1c levels when added to diet and exercise.
• Adjunct therapy in type 2 diabetes: enhances glucose control alongside metformin or sulfonylureas.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the breakdown of the incretin hormones GLP‑1 and GIP. Elevated incretin levels increase glucose‑dependent insulin secretion, suppress glucagon release, and slow gastric emptying, thereby lowering post‑prandial and fasting blood glucose concentrations.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nasal passages and throat.
• Headache: transient mild to moderate head pain.
• Upper respiratory tract infection: symptoms such as cough and sore throat.
• Dizziness: occasional light‑headed sensation.

• Arthralgia: joint pain without swelling.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with elevated enzymes, requiring immediate medical attention.

• Stevens‑Johnson syndrome: rare skin eruption with blistering and mucosal involvement.
• Hepatic injury: abnormal liver function tests indicating potential liver damage.
