Linagliptin Intas (Linagliptin)

Linagliptin is indicated for the management of type 2 diabetes mellitus, a chronic metabolic disorder characterized by hyperglycemia.

• Type 2 diabetes mellitus: improves glycemic control by lowering fasting and post‑prandial blood glucose levels.
• Monotherapy in patients inadequately controlled with diet and exercise alone: provides additional glucose‑lowering effect.
• Combination therapy with metformin or other oral antidiabetics: enhances overall glycemic reduction when used together.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, which degrades the incretin hormones GLP‑1 and GIP. By preventing this degradation, endogenous incretin levels rise, leading to glucose‑dependent stimulation of insulin secretion and suppression of glucagon release, thereby improving post‑prandial and fasting glucose control.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection.
• Headache: transient mild to moderate pain.
• Dizziness: occasional light‑headed sensation.
• Gastrointestinal upset: nausea, abdominal discomfort, or diarrhoea.
• Upper respiratory tract infection: cough or sore throat.
• Skin rash: mild erythema or pruritus.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with vomiting.
• Severe hypersensitivity: angioedema, Stevens‑Johnson syndrome.
• Hepatic injury: elevated transaminases or jaundice.
• Acute renal failure: sudden decline in kidney function.
• Bullous pemphigoid: blistering skin disorder.
• Anaphylactic reaction: rapid onset of airway compromise.

Before initiating Linagliptin, consider patient‑specific factors and monitor appropriately.

• Renal impairment: assess kidney function and adjust therapy if needed.
• Hepatic disease: use with caution in patients with liver dysfunction.
• Elderly patients: monitor for hypoglycaemia when combined with insulin or sulfonylureas.
• Pregnancy and lactation: safety not established; avoid unless benefits outweigh risks.
• Concomitant medications: review for potential drug‑drug interactions.
• Store at room temperature: keep in original packaging, protect from moisture and direct sunlight.

Major Interactions (Avoid)
