Linagliptin Medochemie (Linagliptin)

Linagliptin is a DPP‑4 inhibitor indicated for the management of type 2 diabetes mellitus, either alone or in combination with other antidiabetic agents.

• Type 2 diabetes mellitus (monotherapy): improves glycaemic control by enhancing insulin secretion and reducing glucagon release.
• Type 2 diabetes mellitus (combined with metformin): provides additional glucose‑lowering effect when metformin alone is insufficient.
• Type 2 diabetes mellitus (renal impairment): offers effective glycaemic control without the need for dose adjustment in patients with mild to moderate renal dysfunction.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, which degrades incretin hormones such as GLP‑1 and GIP. By blocking DPP‑4, linagliptin increases endogenous incretin levels, leading to glucose‑dependent stimulation of insulin secretion, suppression of glucagon release, and overall improvement of post‑prandial and fasting blood glucose concentrations.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection symptoms.
• Headache: transient mild to moderate headache.
• Dizziness: occasional light‑headedness.
• Gastrointestinal upset: nausea, abdominal discomfort, or diarrhoea.
• Upper respiratory infection: cough or sore throat.
• Back pain: occasional musculoskeletal discomfort.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with elevated enzymes.
• Severe allergic reactions: angioedema, anaphylaxis, or rash.
• Hepatic injury: jaundice or abnormal liver function tests.
• Severe skin reactions: Stevens‑Johnson syndrome or toxic epidermal necrolysis.
• Hypoglycaemia (when combined with sulfonylureas or insulin): dizziness, sweating, confusion.
• Acute renal failure (rare): reduced urine output and fluid overload.

Linagliptin should be used with caution in the following situations:

• Renal impairment: monitor renal function regularly, especially in patients with chronic kidney disease.
• Hepatic impairment: assess liver enzymes before initiating therapy.
• Concomitant sulfonylureas or insulin: risk of hypoglycaemia, adjust dose as needed.
• Cardiovascular disease: observe for any worsening of heart failure symptoms.
• Pregnancy and breastfeeding: safety not established, avoid unless clearly needed.
• Storage: Store at ambient temperature below 25°C: keep in the original pack, protect from moisture.

Linagliptin may interact with several medicines that affect glucose levels or drug metabolism.

Major Interactions (Avoid)

• Sulfonylureas: increased risk of hypoglycaemia; dose adjustment required.
• Insulin: additive glucose‑lowering effect; monitor blood sugar closely.
• Strong CYP3A4 inducers (e.g., rifampin): may reduce linagliptin exposure and efficacy.

Moderate Interactions (Monitor Closely)

• Strong CYP3A4 inhibitors (e.g., ketoconazole): may increase linagliptin plasma concentrations.
• Diuretics and ACE inhibitors: potential impact on renal function; monitor electrolytes and renal markers.
• NSAIDs: may affect renal clearance; assess renal function periodically.
