Linagliptin is indicated for the management of type 2 diabetes mellitus, a chronic metabolic disorder affecting glucose regulation.

• Type 2 diabetes mellitus: improves blood glucose levels by enhancing insulin secretion and reducing glucagon release.
• Combination therapy with metformin: provides additional glucose‑lowering effect when diet and exercise alone are insufficient.
• Adjunct to diet and exercise: helps patients achieve target HbA1c when lifestyle measures are not enough.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the breakdown of endogenous incretin hormones such as GLP‑1 and GIP. Elevated incretin levels stimulate glucose‑dependent insulin release from pancreatic β‑cells and suppress glucagon secretion from α‑cells, resulting in lowered post‑prandial and fasting plasma glucose concentrations.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nasal passages and throat.
• Headache: transient mild to moderate head pain.
