Linagliptin Medochemie (Linagliptin)

Linagliptin is a DPP‑4 inhibitor used in the treatment of type 2 diabetes mellitus, either alone or in combination with other antidiabetic agents.

• Type 2 diabetes mellitus: improves blood glucose control when added to diet, exercise and other therapies.
• Combination therapy with metformin: provides additional HbA1c reduction beyond metformin alone.
• Adjunct to lifestyle modification: supports overall glycaemic management in patients unable to achieve targets with non‑pharmacologic measures alone.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 enzyme, preventing the degradation of incretin hormones such as GLP‑1 and GIP. Elevated incretin levels enhance glucose‑dependent insulin secretion from pancreatic β‑cells and suppress glucagon release from α‑cells, resulting in lowered post‑prandial and fasting plasma glucose concentrations.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Nasopharyngitis: mild upper respiratory symptoms.
• Headache: transient and usually mild.
• Gastrointestinal upset: includes nausea, abdominal pain or diarrhoea.
• Dizziness: occasional light‑headedness.
• Back pain: musculoskeletal discomfort.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe allergic reactions: rash, angioedema or anaphylaxis.
• Hepatic injury: elevated liver enzymes or jaundice.
• Severe hypoglycaemia: especially when combined with insulin or sulfonylureas.

When prescribing Linagliptin, consider the following precautions.

• Renal impairment: monitor renal function; dose adjustment is generally not required but caution is advised.
• Hepatic impairment: use with caution in patients with moderate to severe liver disease.
• Cardiovascular disease: observe for any worsening of heart failure symptoms.
• Pregnancy: safety not established; avoid unless potential benefit justifies risk.
• Breastfeeding: unknown excretion in milk; consider alternative therapy.
• Store below 25°C: keep in the original packaging, protect from moisture and direct sunlight.

Linagliptin may interact with several medicines; monitor accordingly.

Major Interactions (Avoid)

• Strong P‑glycoprotein inhibitors (e.g., cyclosporine): may increase linagliptin plasma concentrations.
• Insulin secretagogues (e.g., sulfonylureas): heightened risk of hypoglycaemia.

Moderate Interactions (Monitor Closely)

• Rifampin: may reduce linagliptin exposure, potentially decreasing efficacy.
• Diuretics: may affect blood glucose levels, requiring closer monitoring.
• Anticoagulants: occasional reports of altered INR; check coagulation parameters as needed.
