Linagliptin/Metformin is indicated for the management of type 2 diabetes mellitus, addressing hyperglycaemia through complementary mechanisms.

• Type 2 diabetes mellitus: lowers fasting and post‑prandial glucose levels when used together with diet and exercise.
• Inadequate control on metformin alone: adds DPP‑4 inhibition to achieve target HbA1c levels in patients not reaching goals.
• Combination with other oral antidiabetics: provides additive glucose‑lowering effect for patients requiring multi‑drug regimens.
• Adjunct to insulin therapy: enhances glycaemic control when added to basal insulin in advanced disease.

• Linagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), preventing the degradation of endogenous incretin hormones GLP‑1 and GIP, which enhances glucose‑dependent insulin secretion and suppresses glucagon release. Metformin decreases hepatic gluconeogenesis, improves peripheral insulin sensitivity, and increases glucose uptake in muscle, resulting in reduced fasting plasma glucose. The fixed‑dose combination therefore provides complementary actions that lower both fasting and post‑prandial glucose concentrations.

The most frequently reported adverse reactions with Linagliptin/Metformin involve the gastrointestinal system and metabolic disturbances, while rare but serious events have also been documented.
