Linagliptin Pharos (Linagliptin)

Linagliptin Pharos is a DPP-4 inhibitor used in the treatment of type 2 diabetes mellitus.

• Type 2 diabetes mellitus (monotherapy): improves glycemic control in adults when diet and exercise are insufficient.
• Type 2 diabetes mellitus (combination therapy): added to metformin, sulfonylureas, or insulin to achieve target HbA1c.
• Type 2 diabetes mellitus (renal impairment): can be used without dose adjustment in patients with mild to moderate renal impairment.

• Linagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), which degrades the incretin hormones GLP‑1 and GIP. By blocking DPP‑4, linagliptin increases circulating incretin levels, enhancing glucose‑dependent insulin secretion, suppressing glucagon release, and ultimately lowering post‑prandial and fasting blood glucose concentrations.

Common Side Effects

• Nasopharyngitis: mild inflammation of the upper airway.
• Headache: transient, usually mild.
• Upper respiratory tract infection: symptoms of cough and congestion.
• Dizziness: occasional light‑headedness.
• Gastrointestinal discomfort: nausea or abdominal pain.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe hypersensitivity reactions: including angioedema or Stevens‑Johnson syndrome.
• Hepatic injury: elevated liver enzymes or jaundice.
• Bullous pemphigoid: rare autoimmune skin disorder.
• Acute renal failure: very rare, monitor renal function if symptoms arise.
