Linagliptin Sandoz (Linagliptin)

Linagliptin is a DPP‑4 inhibitor used in the management of type 2 diabetes mellitus.

• Type 2 diabetes mellitus (monotherapy): improves glycaemic control as a single oral agent when diet and exercise are insufficient.
• Type 2 diabetes mellitus (combined with metformin): provides additional glucose‑lowering effect in patients already on metformin.
• Type 2 diabetes mellitus (combined with insulin): enhances insulin efficacy and may reduce required insulin dose.

• Linagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), which degrades the incretin hormones glucagon‑like peptide‑1 (GLP‑1) and glucose‑dependent insulinotropic polypeptide (GIP). By blocking DPP‑4, linagliptin raises circulating GLP‑1 and GIP levels, enhancing glucose‑dependent insulin secretion and suppressing glucagon release, thereby lowering both post‑prandial and fasting blood glucose concentrations.

Adverse reactions reported with linagliptin are generally mild, but serious events can occur.

Common Side Effects

• Headache: mild to moderate intensity.
• Nasopharyngitis: symptoms of sore throat or nasal congestion.
• Upper respiratory tract infection: cough, runny nose, or sinus discomfort.
• Gastrointestinal discomfort: nausea, abdominal pain or dyspepsia.
• Dizziness: transient sensation of light‑headedness.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain possibly radiating to the back.
• Severe hypersensitivity reactions: including angioedema, rash or anaphylaxis.
• Hepatic enzyme elevation: significant increase in ALT or AST.
• Bullous pemphigoid: rare autoimmune skin blistering disorder.
• Acute kidney injury: sudden decline in renal function.

Before initiating linagliptin therapy, several safety considerations should be reviewed.

• Renal impairment: dose adjustment is not required, but monitor renal function regularly.
• Hepatic disease: use with caution in patients with moderate hepatic impairment.
• History of pancreatitis: avoid or use only after careful risk‑benefit assessment.
• Concomitant use with insulin or sulfonylureas: increased risk of hypoglycaemia; monitor glucose levels.
• Pregnancy and lactation: safety not established; prescribe only if potential benefit justifies risk.
• Store at ambient temperature: keep below 25 °C in the original packaging, protect from moisture and light.

Linagliptin may interact with other medicines, altering its efficacy or safety profile.

Major Interactions (Avoid)

• Strong CYP3A4 inducers (e.g., rifampicin, carbamazepine): may reduce linagliptin plasma concentrations.
• Combination with other DPP‑4 inhibitors: additive effect increases risk of adverse events.

Moderate Interactions (Monitor Closely)

• Insulin or sulfonylureas: enhanced hypoglycaemia risk; adjust dose as needed.
• Diuretics (e.g., thiazides, loop diuretics): may affect renal clearance of linagliptin.
• Anticoagulants (e.g., warfarin): monitor INR as linagliptin may slightly alter anticoagulant effect.
• ACE inhibitors or ARBs: no clinically significant interaction but monitor blood pressure and renal function.
