Linagliptin Stada (Linagliptin)

Linagliptin is a DPP‑4 inhibitor used in the treatment of type 2 diabetes mellitus, acting on the incretin pathway to improve glucose regulation.

• Type 2 diabetes mellitus: improves blood glucose control when combined with diet and exercise.
• Inadequate control with metformin alone: provides additional glycaemic lowering as an adjunct therapy.
• Combination therapy: can be used with other antidiabetic agents such as sulfonylureas or insulin to achieve target HbA1c levels.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the breakdown of endogenous incretin hormones (GLP‑1 and GIP). Elevated incretin levels enhance glucose‑dependent insulin secretion from pancreatic β‑cells and suppress glucagon release from α‑cells, resulting in lowered post‑prandial and fasting plasma glucose concentrations.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection symptoms.
• Headache: transient mild to moderate headache.
• Upper respiratory infection: cough, sore throat, or congestion.
• Gastrointestinal upset: nausea, abdominal discomfort, or diarrhoea.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain possibly radiating to the back.
• Severe allergic reactions: rash, angio‑edema, or anaphylaxis.
• Hepatic injury: elevated liver enzymes or jaundice.
• Bullous pemphigoid: blistering skin disorder.
• Severe hypoglycaemia: when combined with insulin or sulfonylureas.

Before initiating Linagliptin, consider patient‑specific factors and monitor for potential risks.

• Renal impairment: dose adjustment is not required, but monitor renal function regularly.
• Hepatic impairment: use with caution in patients with moderate to severe liver disease.
• Cardiovascular disease: assess cardiovascular status as part of overall diabetes management.
• Elderly patients: monitor for adverse events, as they may be more susceptible.
• Pregnancy and breastfeeding: safety not established; avoid unless benefits outweigh risks.
