Linagliptin is a DPP‑4 inhibitor used in the management of type 2 diabetes mellitus.

• Monotherapy in type 2 diabetes mellitus: Improves fasting and post‑prandial blood glucose levels.
• Combination therapy with metformin or other antidiabetics: Enhances overall glycemic control when added to existing regimens.
• Adjunct to lifestyle modification: Supports blood glucose management when diet and exercise alone are insufficient.

• Linagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), which degrades the incretin hormones GLP‑1 and GIP. By blocking DPP‑4, linagliptin prolongs the activity of these hormones, leading to increased insulin secretion from pancreatic β‑cells and reduced glucagon release from α‑cells in a glucose‑dependent manner, thereby improving overall glycemic control without causing hypoglycaemia when used alone. The effect is independent of renal function, allowing use without dose adjustment in patients with renal impairment.

Linagliptin may be associated with several adverse reactions, ranging from mild to serious.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nose and throat.
• Headache: transient head pain.
• Dizziness: sensation of light‑headedness.
• Gastrointestinal discomfort: nausea, abdominal pain or diarrhea.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain with elevated enzymes.
• Severe allergic reaction: rash, swelling, or anaphylaxis.
• Bullous pemphigoid: rare skin blistering disorder.

When prescribing linagliptin, clinicians should consider several precautionary measures to ensure safe use.

• Renal impairment: evaluate kidney function; dose adjustment is generally not required but monitor.
• Hepatic impairment: use with caution and monitor liver enzymes.
• Cardiovascular disease: assess risk, especially in patients with heart failure.
• Pregnancy and lactation: safety not established; avoid unless benefits outweigh risks.
• Concomitant sulfonylureas or insulin: increased risk of hypoglycaemia; monitor glucose.
• Store at ambient temperature: keep below 25 °C in the original packaging, protect from moisture and light.
