Linagliptin Tzf (Linagliptin)

Linagliptin is a DPP‑4 inhibitor indicated for the management of type 2 diabetes mellitus.

• Type 2 diabetes mellitus: improves glycaemic control as monotherapy or in combination with other antidiabetic agents.
• Adjunct to diet and exercise: helps patients achieve target HbA1c levels when lifestyle measures alone are insufficient.
• Patients with renal impairment: can be used without dose adjustment, providing a convenient option for those with reduced kidney function.

• Linagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), which degrades the incretin hormones GLP‑1 and GIP. By preventing this degradation, linagliptin increases endogenous incretin levels, enhancing glucose‑dependent insulin secretion from pancreatic β‑cells and suppressing glucagon release from α‑cells, thereby lowering blood glucose concentrations.

Linagliptin is generally well tolerated, but several adverse reactions have been reported.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection symptoms.
• Headache: transient mild to moderate headache.
• Gastrointestinal discomfort: occasional nausea, abdominal pain or dyspepsia.
• Dizziness: occasional light‑headedness, especially on standing.
• Upper respiratory infection: cough or sore throat.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe allergic reactions: rash, angioedema or anaphylaxis.
• Hepatic injury: elevated liver enzymes or jaundice.
• Hypersensitivity: serious skin reactions such as Stevens‑Johnson syndrome.

Linagliptin should be used with awareness of specific safety considerations.

• Renal function: monitor renal status, although dose adjustment is not required.
• Hepatic impairment: use with caution in patients with liver disease.
