Linagliptin Zentiva (Linagliptin)

Linagliptin is a DPP‑4 inhibitor used in the management of type 2 diabetes mellitus.

• Type 2 diabetes mellitus: Improves glycemic control as monotherapy or in combination with other antidiabetic agents.
• Adjunct therapy with metformin: Enhances overall blood glucose reduction when added to metformin therapy.
• Combination with insulin: Provides additional HbA1c lowering effect while reducing insulin dose requirements.
• Patients inadequately controlled on diet alone: Offers an oral option to achieve target glucose levels.

• Linagliptin selectively inhibits the enzyme dipeptidyl peptidase‑4 (DPP‑4), which degrades the incretin hormones GLP‑1 and GIP. By blocking DPP‑4, linagliptin increases endogenous incretin concentrations, leading to glucose‑dependent stimulation of insulin secretion, suppression of glucagon release, and delayed gastric emptying. This results in improved post‑prandial and fasting glucose levels without causing hypoglycaemia when used alone.

Linagliptin may be associated with a range of adverse reactions, most of which are mild and transient.

Common Side Effects

• Nasopharyngitis: Inflammation of the nasal passages and throat, usually mild and self‑limiting.
• Headache: Mild to moderate head pain that may improve with rest or analgesics.
• Diarrhoea: Loose stools occurring intermittently, often without dehydration.

Serious or Rare Side Effects

• Pancreatitis: Severe abdominal pain with elevated pancreatic enzymes, requiring immediate medical attention.
• Severe hypersensitivity reactions: Including angioedema, widespread rash, or anaphylaxis, which can be life‑threatening.
• Bullous skin eruptions: Rare blistering skin conditions such as Stevens‑Johnson syndrome.

When prescribing linagliptin, clinicians should consider several safety considerations to minimise risk.

• Renal impairment: Use with caution; monitor renal function and adjust dose if necessary.
• Hepatic impairment: Assess liver enzymes before initiation and monitor periodically.
• Elderly patients: Evaluate comorbidities and concomitant medications due to increased sensitivity.
• Concomitant insulin or sulfonylureas: Monitor for signs of hypoglycaemia and adjust therapy accordingly.
• Pregnancy and lactation: Use only if the potential benefit justifies the potential risk to the fetus or infant.
• Store at ambient temperature: Keep below 25 °C, retain in the original packaging, and protect from moisture.

Linagliptin can interact with other medicines, influencing its efficacy or safety.

Major Interactions (Avoid)

• Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole): May increase linagliptin plasma concentrations.
• Strong CYP3A4 inducers (e.g., rifampicin, carbamazepine): May reduce linagliptin exposure and effectiveness.
• Insulin or sulfonylureas: Combined use can heighten the risk of hypoglycaemia.

Moderate Interactions (Monitor Closely)

• Diuretics: May affect glucose homeostasis; monitor blood sugar levels.
• Antihypertensive agents: Potential additive blood pressure effects; observe patient response.
• Oral contraceptives: May slightly alter linagliptin metabolism; assess therapeutic outcomes.
