Linagliptin Zentiva (Linagliptin)

Linagliptin is a DPP‑4 inhibitor indicated for the management of type 2 diabetes mellitus.

• Type 2 diabetes mellitus: improves blood glucose control when used with diet and exercise.
• Combination therapy: enhances glycaemic control when added to metformin or other antidiabetic agents.
• Adjunct to lifestyle modification: supports maintenance of target HbA1c levels in adult patients.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the degradation of incretin hormones such as GLP‑1 and GIP. Elevated incretin levels increase glucose‑dependent insulin secretion and suppress glucagon release, thereby reducing post‑prandial and fasting plasma glucose concentrations in patients with type 2 diabetes.

Linagliptin is generally well tolerated, but adverse reactions can occur.

Common Side Effects

• Upper respiratory tract infection: mild symptoms such as cough or sore throat.
• Headache: transient and usually mild.
• Nasopharyngitis: inflammation of the nasal passages and throat.
• Diarrhoea: occasional loose stools.
• Nausea: occasional feeling of queasiness.
• Skin rash: mild erythema or itching.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe hypersensitivity: angioedema or anaphylactic reactions.
• Hepatic dysfunction: elevated liver enzymes indicating possible liver injury.
• Acute kidney injury: sudden decline in renal function, especially in patients with pre‑existing kidney disease.
• Heart failure exacerbation: worsening symptoms in susceptible individuals.

Before initiating Linagliptin, consider patient‑specific factors and monitor appropriately.

• Renal impairment: dose adjustment may be required in severe renal dysfunction.
• Hepatic disease: use with caution in patients with liver impairment.
• Cardiovascular risk: monitor for signs of heart failure, especially in high‑risk patients.
• Pregnancy and lactation: safety not established; avoid unless benefits outweigh risks.
• Drug interactions: review concomitant medications for potential interactions.
• Store at ambient temperature: keep below 25 °C, in the original packaging, protected from moisture and direct sunlight.

Major Interactions (Avoid)

• Strong CYP3A4 inducers (e.g., rifampicin): may reduce linagliptin plasma concentrations.
• Combination with other DPP‑4 inhibitors: increased risk of adverse effects.

• Diuretics and antihypertensives: may enhance risk of dehydration and hypotension.
• Insulin or sulfonylureas: increased risk of hypoglycaemia; adjust dose as needed.
• NSAIDs: may affect renal function when used together in susceptible patients.
