Linagliptin is a DPP‑4 inhibitor indicated for the management of type 2 diabetes mellitus, acting on the incretin pathway to lower blood glucose.

• Type 2 diabetes mellitus (monotherapy): improves fasting and post‑prandial glucose levels.
• Type 2 diabetes mellitus (add‑on to metformin): provides additional glycaemic reduction when metformin alone is insufficient.
• Type 2 diabetes mellitus with renal impairment: offers effective control without dose adjustment in patients with reduced kidney function.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 enzyme, preventing the breakdown of endogenous incretin hormones (GLP‑1 and GIP). Elevated incretin levels enhance glucose‑dependent insulin secretion from pancreatic β‑cells and suppress glucagon release from α‑cells, resulting in lowered plasma glucose concentrations.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract infection symptoms.
• Headache: transient mild to moderate headache.
• Dizziness: occasional light‑headed sensation.
