Linagliptina Generis Phar (Linagliptin)

• Linagliptin is a DPP-4 inhibitor used in the management of type 2 diabetes mellitus.
• Type 2 diabetes mellitus (monotherapy): achieves improved glycaemic control when diet and exercise alone are insufficient.
• Combination with metformin: provides additional HbA1c reduction in patients inadequately controlled on metformin alone.
• Adjunct to other oral antidiabetics: enhances overall glucose lowering when added to sulfonylureas, thiazolidinediones or SGLT2 inhibitors.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, preventing the degradation of endogenous incretin hormones such as GLP‑1 and GIP. Elevated incretin levels increase glucose‑dependent insulin secretion and suppress glucagon release, resulting in lowered post‑prandial and fasting blood glucose concentrations.

Linagliptin may cause a range of adverse reactions, varying from mild to severe.

Common Side Effects

• Nasopharyngitis: mild upper respiratory tract inflammation.
• Headache: transient mild to moderate pain.
• Upper respiratory infection: symptoms of cough and congestion.
• Dizziness: occasional light‑headedness.
• Gastrointestinal discomfort: mild nausea or abdominal pain.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe hypersensitivity reactions: including Stevens‑Johnson syndrome or toxic epidermal necrolysis.
• Hepatic enzyme elevation: significant increase in ALT/AST levels.
• Angioedema: swelling of face, lips, or airway.
• Bullous pemphigoid: rare autoimmune blistering skin disease.
• Anaphylaxis: life‑threatening allergic response.

• Before initiating linagliptin therapy, consider the following precautions.
• Renal impairment: no dose adjustment is required, but monitor renal function regularly.
• Hepatic impairment: use with caution and assess liver enzymes periodically.
• History of pancreatitis: discontinue if signs of pancreatitis develop.
• Hypersensitivity reactions: stop the drug immediately if rash, angioedema or anaphylaxis occur.
• Pregnancy and lactation: use only if the potential benefit outweighs the potential risk to the fetus or infant.
