Linathan (Linagliptin)

Linathan is a DPP‑4 inhibitor used in the management of type 2 diabetes mellitus, either alone or in combination with other antidiabetic agents.

• Type 2 diabetes mellitus (monotherapy): provides glycaemic control as a standalone treatment.
• Type 2 diabetes mellitus (combination therapy): used together with metformin or other agents to enhance glucose lowering.
• Type 2 diabetes mellitus (adjunct to diet and exercise): supports blood‑sugar management when lifestyle measures alone are insufficient.

• Linagliptin selectively inhibits the dipeptidyl peptidase‑4 (DPP‑4) enzyme, which results in increased levels of endogenous incretin hormones (GLP‑1 and GIP). The elevated incretins enhance glucose‑dependent insulin secretion from pancreatic β‑cells and suppress glucagon release from α‑cells, thereby lowering post‑prandial and fasting plasma glucose concentrations.

Common Side Effects

• Nasopharyngitis: mild inflammation of the nasal passages and throat.
• Headache: transient mild to moderate head pain.
• Urinary tract infection: symptoms of dysuria or frequency.
• Back pain: occasional musculoskeletal discomfort.
• Dizziness: brief episodes of light‑headedness.
• Gastrointestinal upset: nausea or abdominal discomfort.

Serious or Rare Side Effects

• Pancreatitis: severe abdominal pain requiring immediate medical attention.
• Severe allergic reactions: angioedema, Stevens‑Johnson syndrome, or anaphylaxis.
• Hepatic injury: elevated liver enzymes or jaundice.
• Severe hypoglycaemia: especially when combined with insulin or sulfonylureas.
• Acute renal failure: rare but reported in vulnerable patients.
• Cardiovascular events: rare cases of myocardial infarction.

Linathan should be used with caution in patients with renal or hepatic impairment, and monitoring is advised for hypoglycaemia when combined with insulin or sulfonylureas.

• Renal impairment: assess renal function regularly; dose adjustment is not required but caution is advised.
• Hepatic impairment: use with caution; monitor liver enzymes.
• Hypersensitivity: discontinue if signs of allergic reaction appear.
• Pregnancy and lactation: safety not established; use only if benefits outweigh risks.
• Concomitant sulfonylureas or insulin: increased risk of hypoglycaemia; monitor glucose levels closely.
• Store at ambient temperature below 25°C: keep in the original pack, protect from moisture and light.

Linathan may interact with several drug classes; clinicians should review patient medication lists before initiating therapy.

Major Interactions (Avoid)

• Other DPP‑4 inhibitors: duplicate inhibition may increase adverse‑event risk.
• Strong P‑glycoprotein inhibitors (e.g., itraconazole): may raise linagliptin plasma concentrations.
• Insulin or sulfonylureas: heightened hypoglycaemia risk; avoid uncontrolled combination.

Moderate Interactions (Monitor Closely)

• Rifampin: may reduce linagliptin exposure; monitor glycaemic response.
• Diuretics: monitor renal function as volume changes can affect drug handling.
• ACE inhibitors: watch for additive hypotensive effects, especially in volume‑depleted patients.
