Micafungin Pharmazac (Micafungin)

Micafungin is an echinocandin antifungal indicated for several serious Candida infections.

• Invasive candidiasis: provides effective treatment of bloodstream and deep‑tissue Candida infections, reducing mortality in immunocompromised patients.
• Esophageal candidiasis: eradicates Candida infection of the esophagus, relieving dysphagia and pain in at‑risk individuals.
• Prophylaxis of Candida infections in hematopoietic stem cell transplantation: prevents invasive Candida disease during the neutropenic phase of transplant conditioning.
• Candida peritonitis and intra‑abdominal infections: used to treat Candida involvement in peritoneal and intra‑abdominal sites, often in combination with other antimicrobials.

• Micafungin selectively inhibits the fungal enzyme β‑(1,3)‑D‑glucan synthase, which catalyzes the synthesis of β‑(1,3)‑D‑glucan, an essential polymer of the fungal cell wall. By blocking this enzyme, micafungin disrupts cell wall construction, leading to osmotic instability and cell death. The drug exhibits fungicidal activity against most Candida species and limited activity against Aspergillus species, making it valuable for treating invasive candidiasis.

Micafungin may be associated with a range of adverse reactions, varying from mild to severe.

Common Side Effects

• Elevated liver enzymes: transient increase in ALT or AST.
• Fever: mild to moderate temperature rise.
• Nausea: occasional gastrointestinal discomfort.

Serious or Rare Side Effects

• Hepatotoxicity: significant liver injury requiring monitoring.
• Severe hypersensitivity reactions: anaphylaxis or angioedema.
• Infusion‑related reactions: hypotension or bronchospasm.

When using micafungin, clinicians should observe several precautionary measures to minimize risk.

• Monitor liver function: assess ALT, AST and bilirubin before and during therapy.
• Assess renal function: evaluate serum creatinine in patients with pre‑existing kidney disease.
• Use cautiously in patients with known hypersensitivity to echinocandins.
• Adjust therapy in severe hepatic impairment: consider dose modification or alternative agents.
• Avoid concomitant use with strong CYP3A4 inducers without monitoring.
• Store at 2‑8 °C: keep refrigerated, protect from light, do not freeze or shake.

Micafungin can interact with other medicines, affecting its efficacy or safety.

Major Interactions (Avoid)

• Cyclosporine: increased micafungin exposure may raise toxicity risk.
• Rifampin: strong enzyme induction may markedly reduce micafungin plasma levels.

• Azole antifungals (e.g., fluconazole): may increase micafungin concentrations.
