Indications & Clinical Uses

Mycophenolic Acid is used to modulate immune responses in conditions where excessive lymphocyte activity is harmful.

  • Organ transplant rejection: helps prevent the immune system from attacking the transplanted organ.
  • Lupus nephritis: reduces kidney inflammation caused by systemic lupus erythematosus.
  • Vasculitis: diminishes inflammation of blood vessels in various autoimmune vasculitic disorders.

How It Works

  • Mycophenolic Acid selectively and reversibly inhibits inosine monophosphate dehydrogenase (IMPDH), a key enzyme in the de novo synthesis of guanosine nucleotides. This inhibition lowers intracellular guanosine levels, impairing DNA synthesis and proliferation of activated T and B lymphocytes, thereby exerting immunosuppressive effects without markedly affecting other cell types.

Side Effects

Mycophenolic Acid may cause a range of adverse reactions, which are generally categorized by frequency and severity.

Common Side Effects

  • Gastrointestinal upset: nausea, vomiting, diarrhea, or abdominal pain.
  • Headache: mild to moderate intensity.
  • Infections: increased susceptibility to upper respiratory or urinary tract infections.
  • Anemia: reduced red blood cell count causing fatigue.
  • Elevated liver enzymes: mild transaminase elevations.
  • Rash: mild skin irritation or itching.

Serious or Rare Side Effects

  • Bone marrow suppression: severe leukopenia, neutropenia, or thrombocytopenia.
  • Opportunistic infections: including fungal or viral infections such as cytomegalovirus.
  • Malignancies: increased risk of lymphoproliferative disorders.
  • Progressive multifocal leukoencephalopathy (PML): rare but potentially fatal brain infection.
  • Hepatotoxicity: significant liver injury or hepatitis.
  • Severe hypersensitivity reactions: including Stevens‑Johnson syndrome.

Precautions & Warnings

Before initiating therapy, clinicians should assess baseline laboratory values and monitor patients regularly.

  • Monitor blood counts: check complete blood count periodically to detect bone marrow suppression.
  • Assess renal function: adjust dosing in patients with impaired kidney function.
  • Avoid in pregnancy: drug is contraindicated due to teratogenic risk.
  • Use effective contraception: both male and female patients of reproductive potential must employ reliable birth control.
  • Review concomitant medications: avoid drugs that may increase mycophenolic acid exposure.
  • Store at ambient temperature: keep below 25°C in the original packaging, protect from moisture.

Avoid Interactions

Mycophenolic Acid can interact with several medicines, altering its efficacy or safety profile.

Major Interactions (Avoid)

  • Cyclosporine, tacrolimus: may increase mycophenolic acid exposure and toxicity.
  • Live vaccines: heightened risk of severe infection due to immunosuppression.
  • Antacids containing aluminum or magnesium: can reduce oral absorption of the drug.

Moderate Interactions (Monitor Closely)

  • Rifampin: may lower mycophenolic acid plasma levels, reducing effectiveness.
  • Warfarin: potential alteration of anticoagulant effect, requiring INR monitoring.
  • Cholestyramine: may decrease drug exposure by binding in the gut.
