Octaplex (Human Coagulation Factor Ix, Protein C, Human Coagulation Factor Vii, Protein S, Human Coagulation F)

Octaplex is used to replace multiple deficient coagulation factors in patients with inherited or acquired bleeding disorders.

• Hemophilia A and B with inhibitors: provides temporary hemostasis while awaiting specific factor replacement.
• Congenital protein C deficiency: restores protein C activity to reduce thrombotic risk.
• Congenital protein S deficiency: supplies protein S to aid regulation of coagulation.
• Peri‑operative management of major surgery: supports hemostasis in patients with multiple factor deficiencies.
• Acquired coagulopathy during liver‑transplant or massive trauma: contributes to balanced coagulation restoration.

• Octaplex contains a mixture of human coagulation factors (II, VII, IX, X) and regulatory proteins (Protein C, Protein S). When administered intravenously, the concentrate directly supplements the missing clotting proteins, enabling the intrinsic and extrinsic pathways of the coagulation cascade to generate thrombin and form a stable fibrin clot, thereby correcting the hemostatic defect.

Adverse reactions to Octaplex are generally related to its protein nature and the intravenous route.

Common Side Effects

• Fever or chills
• Mild headache
• Nausea or vomiting
• Transient hypotension
• Local infusion site erythema
• Mild allergic skin rash

Serious or Rare Side Effects

• Anaphylactic or severe hypersensitivity reaction
• Thromboembolic events (deep‑vein thrombosis, pulmonary embolism)
• Disseminated intravascular coagulation (DIC)
• Hemolysis or renal impairment
• Development of neutralising antibodies against the infused factors
• Severe sepsis from contaminated product

Octaplex should be administered under close clinical supervision with attention to patient‑specific risk factors.

• Prior exposure assessment: review the patient history for previous reactions to plasma‑derived products.
• Thrombotic risk evaluation: assess the patient for underlying pro‑thrombotic conditions.
• Monitoring: conduct regular laboratory tests for coagulation parameters and renal function.
• Pregnancy and liver disease :
