Indications & Clinical Uses

Octostim is a vasopressin analog that acts on the renal collecting ducts and coagulation pathways.

  • Central diabetes insipidus: helps control excessive urine output and thirst.
  • Nocturnal enuresis: reduces nighttime bedwetting episodes in children.
  • Mild hemophilia A and von Willebrand disease: raises plasma levels of factor VIII and von Willebrand factor to improve clotting.

How It Works

  • Desmopressin acetate binds selectively to V2 receptors in the renal collecting ducts, stimulating the insertion of aquaporin‑2 water channels and promoting water reabsorption. It also triggers the release of stored factor VIII and von Willebrand factor from endothelial cells, enhancing primary haemostasis.

Side Effects

Desmopressin may cause a range of adverse reactions, most of which are mild and transient.

Common Side Effects

  • Headache: mild to moderate intensity.
  • Nasal congestion: sensation of a blocked nose.
  • Nausea: occasional upset stomach.
  • Hyponatremia: low sodium levels, usually mild.
  • Flushing: brief redness of the skin.
  • Mild dizziness: light‑headed feeling.

Serious or Rare Side Effects

  • Severe hyponatremia: can lead to seizures or coma.
  • Thromboembolic events: blood clots in veins or arteries.
  • Allergic reaction: rash, itching, swelling, or difficulty breathing.
  • Hypertension: significant rise in blood pressure.
  • Cardiac arrhythmia: irregular heart rhythm.
  • Pulmonary edema: fluid accumulation in the lungs.

Precautions & Warnings

When prescribing Octostim, clinicians should consider several safety aspects.

  • Monitor sodium: check serum sodium before treatment and periodically during therapy.
  • Fluid restriction: advise patients to limit fluid intake to reduce hyponatremia risk.
  • Renal function: assess kidney function, especially in the elderly.
  • Concomitant drugs: review use of other antidiuretic agents or NSAIDs.
  • Cardiovascular status: evaluate for hypertension or heart failure.
  • Store at ambient temperature: keep below 25°C, in the original packaging, protected from moisture.
