Indications & Clinical Uses

Octreotide is a synthetic analogue of somatostatin that modulates neuroendocrine activity in several disease pathways.

  • Acromegaly: reduces growth hormone and IGF‑1 levels, helping control disease activity.
  • Neuroendocrine tumors (carcinoid syndrome): alleviates flushing and diarrhoea associated with hormone secretion.
  • Variceal bleeding prophylaxis: lowers portal pressure and reduces risk of re‑bleeding.
  • Severe hypoglycaemia: suppresses insulin release in patients with refractory hypoglycaemia.
  • Diagnostic imaging: enhances contrast of somatostatin‑receptor positive lesions during scintigraphy.

How It Works

  • Octreotide binds selectively to somatostatin receptor subtypes (mainly sst‑2 and sst‑5) on endocrine and neuroendocrine cells, inhibiting cyclic AMP formation and calcium influx. This receptor activation suppresses the secretion of several hormones—including growth hormone, insulin, glucagon, and various gastrointestinal peptides—thereby moderating the clinical manifestations of hormone‑driven disorders.

Side Effects

Adverse reactions to octreotide are generally dose‑related and may vary with the route of administration.

Common Side Effects

  • Gastrointestinal discomfort: nausea, abdominal cramping, or diarrhoea.
  • Injection site reactions: erythema, pain or induration.
  • Gallbladder changes: biliary sludge or gallstones.
  • Metabolic alterations: hyperglycaemia or hypoglycaemia.
  • Headache or fatigue.

Serious or Rare Side Effects

  • Pancreatitis.
  • Severe hyperglycaemia requiring insulin therapy.
  • Acute cholecystitis.
  • Cardiac arrhythmias.
  • Anaphylactic‑type reactions.
  • Severe injection‑site infection.

Precautions & Warnings

Octreotide should be used only under medical supervision, with attention to patient‑specific factors and monitoring requirements.

  • Renal impairment: dose may need adjustment and renal function should be monitored.
  • Hepatic dysfunction: use caution and assess liver enzymes regularly.
  • Diabetes mellitus: monitor blood glucose closely as the drug can affect glycaemic control.
  • Gallbladder disease: patients with a history of gallstones should be evaluated before therapy.
  • Pregnancy and lactation: only use if the potential benefit outweighs potential risk to fetus or infant.
  • Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Avoid Interactions

Octreotide may interact with several medicines, altering either its efficacy or the effect of the co‑administered drug.

Major Interactions (Avoid)

  • Cytochrome P450 inducers: carbamazepine, rifampicin – may reduce octreotide plasma levels.
  • Long‑acting somatostatin analogues: concurrent use can increase risk of gallbladder complications.

Moderate Interactions (Monitor Closely)

  • Insulin and oral hypoglycaics: octreotide can blunt insulin secretion, requiring dose adjustments.
  • Antiplatelet agents: may increase bleeding time when used with octreotide for variceal bleeding prophylaxis.
  • Diuret‑s‑for‑heart    ---
