Octreotide Bioindustria L.I.M. (Octreotide)

Octreotide acts on neuroendocrine pathways and is used in several clinically recognised indications:

• Acromegaly: reduces growth hormone and IGF‑1 levels, helping control disease activity.
• Carcinoid syndrome: lessens flushing and diarrhoea by inhibiting serotonin release.
• VIPoma-associated diarrhea: improves symptoms by blocking VIP‑mediated intestinal secretion.
• Preventing postoperative pancreatic fistula after pancreatic surgery : reduces the risk of fistula development .
• Control of bleeding from gastro‑intestinal tumours : provides haemostatic control when conventional methods fail .

• Octreotide mimics natural somatostatin, binding to somatostatin receptors (mainly SSTR‑2 and SSTR‑5) on target cells. This interaction inhibits adenylate cyclase activity, reduces cyclic AMP formation, and suppresses the release of several hormones and peptides such as growth hormone, insulin, glucagon, gastrin, and serotonin, thereby modulating endocrine and exocrine secretions.

Octreotide may cause a range of adverse reactions, most of which are mild to moderate in intensity.

Common Side Effects

• Nausea: mild to moderate gastrointestinal upset.
• Abdominal pain: cramping or discomfort in the abdomen.
• Diarrhoea: loose stools occurring intermittently.
• Injection site reactions: redness, swelling or bruising at the site of administration.
• Headache: transient mild headache.
• Hyperglycaemia: elevated blood glucose levels, particularly in diabetic patients.

Serious or Rare Side Effects

• Gallbladder disease: formation of gallstones or biliary sludge.
• Pancreatitis: inflammation of the pancreas.
• Severe hypoglycaemia: marked low blood glucose, especially with concomitant insulin therapy.
• Allergic reactions: rash, itching, swelling, or anaphylaxis.
• Cardiac arrhythmias: irregular heart rhythms in susceptible individuals.
• Acute renal failure: sudden loss of kidney function.

When using Octreotide, clinicians should consider several safety aspects:

• Monitor glucose: regular blood‑glucose monitoring is advised, especially in diabetic patients.
• Assess gallbladder: periodic imaging may be warranted for patients on long‑term therapy.
• Evaluate cardiac function: patients with known arrhythmias should be observed.
• Review concomitant meds: check for drugs that affect glucose or gastrointestinal motility.
• Adjust in renal impairment: dose may need modification in severe renal dysfunction.
