Indications & Clinical Uses

Omeprazole is a proton pump inhibitor that acts on the gastric acid secretion pathway.

  • Gastro‑esophageal reflux disease (GERD): Improves symptom control and promotes healing of the esophageal lining.
  • Peptic ulcer disease: Facilitates ulcer healing and reduces recurrence.
  • Zollinger‑Ellison syndrome: Controls excessive gastric acid production.
  • NSAID‑induced gastropathy: Provides gastro‑protection.
  • Helicobacter pylori eradication regimens: Used as part of combination therapy to enhance eradication rates.
  • Stress‑related ulcer prophylaxis: Reduces risk of stress‑related mucosal damage.

How It Works

  • Omeprazole irreversibly binds to the H+/K+‑ATPase enzyme (the gastric proton pump) on the secretory surface of parietal cells, inhibiting the final step of acid secretion. This results in a prolonged reduction of gastric acidity, allowing mucosal healing and symptom relief.

Side Effects

Omeprazole is generally well‑tolerated, but adverse reactions can occur.

Common Side Effects

  • Headache: Mild to moderate headache.
  • Diarrhea: Loose stools, often transient.
  • Abdominal pain: Discomfort or cramping in the stomach area.
  • Nausea: Feeling of queasiness.
  • Flatulence: Excessive gas.
  • Constipation: Reduced bowel movements.

Serious or Rare Side Effects

  • Clostridioides difficile infection: Severe colitis presenting with watery diarrhea.
  • Acute interstitial nephritis: Inflammation of kidney interstitium causing renal impairment.
  • Hypomagnesemia: Low serum magnesium leading to muscle cramps or arrhythmias.
  • Stevens‑Johnson syndrome: Severe skin reaction with blistering.
  • Liver enzyme elevation: Increased ALT/AST levels indicating hepatic stress.
  • Bone fracture risk: Potential reduction in calcium absorption with long‑term use.

Precautions & Warnings

When using Omeprazole, consider the following precautions:

  • Assess renal function: Adjust use in patients with severe renal impairment.
  • Monitor magnesium levels: Long‑term therapy may deplete magnesium.
  • Evaluate bone health: Prolonged use may affect calcium absorption.
  • Review concomitant medications: Potential for altered absorption of drugs requiring acidic pH.
  • Pregnancy and lactation: Use only if clearly needed.
  • Store at below 20°C: Keep in the original packaging.
