Indications & Clinical Uses

Omeprazole acts on the gastric acid secretion pathway and is indicated for several acid‑related disorders.

  • Gastro‑oesophageal reflux disease (GERD): provides symptomatic relief and promotes healing of erosive esophagitis.
  • Peptic ulcer disease: supports ulcer healing and reduces risk of recurrence.
  • Zollinger‑Ellison syndrome: controls excessive gastric acid secretion.
  • Erosive gastritis: aids mucosal healing and symptom control.
  • NSAID‑associated ulcer prophylaxis: lowers incidence of ulcer development in at‑risk patients.
  • Helicobacter pylori eradication regimens (adjunct): enhances eradication success when combined with antibiotics.

How It Works

  • Omeprazole is a pro‑drug that is converted in the acidic environment of the secretory canaliculi of parietal cells to its active sulphenamide form. The active molecule binds covalently and irreversibly to the H+/K+‑ATPase (the gastric proton pump) on the luminal surface of parietal cells, inhibiting the final step of acid secretion and thereby markedly lowering gastric acidity.

Side Effects

Omeprazole is generally well tolerated, but a range of adverse reactions have been reported.

Common Side Effects

  • Headache: mild to moderate intensity.
  • Diarrhea: typically self‑limiting.
  • Abdominal pain or discomfort: occasional.
  • Nausea or vomiting: transient.
  • Flatulence: mild.
  • Constipation: occasional.

Serious or Rare Side Effects

  • Clostridioides difficile infection: may occur with prolonged use.
  • Severe hypersensitivity reactions: including Stevens‑Johnson syndrome.
  • Acute interstitial nephritis: rare renal complication.
  • Osteoporotic fractures: increased risk with long‑term therapy.
  • Hypomagnesemia: low serum magnesium with extended use.
  • Vitamin B12 deficiency: possible with chronic suppression of acid.

Precautions & Warnings

Omeprazole should be used with consideration of several safety aspects.

  • Assess renal function: dose adjustment may be required in severe renal impairment.
  • Monitor for infections: prolonged acid suppression can increase risk of gastrointestinal infections.
  • Evaluate bone health: long‑term use may affect calcium absorption.
  • Review concomitant medications: potential for altered absorption of drugs that require an acidic environment.
  • Pregnancy and lactation: use only if clearly needed; discuss risks with a healthcare professional.
  • Store at ambient temperature: keep in the original packaging, protect from moisture and direct sunlight.

Avoid Interactions

Omeprazole can affect the pharmacokinetics of several co‑administered medicines.

Major Interactions (Avoid)

  • Ketoconazole or itraconazole: reduced antifungal efficacy due to decreased gastric acidity.
  • Atazanavir: markedly lowered plasma concentrations, compromising HIV therapy.
  • Clopidogrel: diminished antiplatelet effect, increasing cardiovascular risk.

Moderate Interactions (Monitor Closely)

  • Warfarin: potential alteration of INR, requiring closer monitoring.
  • Phenytoin: may increase serum levels, necessitating dosage review.
  • Digoxin: enhanced absorption may raise serum digoxin concentrations.
