Indications & Clinical Uses

Omeprazole is a proton pump inhibitor that acts on the gastric acid secretion pathway. It is indicated for several acid‑related disorders:

  • Gastro‑esophageal reflux disease (GERD): relief of heartburn and healing of erosive esophagitis.
  • Peptic ulcer disease (PUD): promotes ulcer healing and reduces recurrence.
  • Zollinger‑Ellison syndrome: controls gastric acid hypersecretion.
  • Helicobacter pylori eradication regimens: used as adjunct adjunctive therapy in combination regimens.
  • NSAID‑associated ulcer prophylaxis: reduces risk of ulcer formation in at‑risk patients.

How It Works

  • Omeprazole irreversibly binds to the H+/K+‑ATPase enzyme (the gastric proton pump) on the secretory surface of parietal cells, blocking the final step of acid secretion. This inhibition markedly lowers gastric acidity, allowing mucosal healing and symptom relief in acid‑related disorders.

Side Effects

Omeprazole is generally well tolerated, but adverse reactions can occur.

Common Side Effects

  • Headache: mild, transient headache.
  • Diarrhea: loose stools, often self‑limiting.
  • Nausea: occasional feeling of queasiness.
  • Abdominal pain: mild discomfort in the stomach area.
  • Flatulence: increased gas production.
  • Constipation: reduced bowel movements.

Serious or Rare Side Effects

  • Severe allergic reaction: rash, itching, swelling, dizziness, difficulty breathing.
  • Clostridioides difficile infection: persistent watery diarrhea.
  • Low magnesium levels: muscle cramps, irregular heartbeat.
  • Bone fracture risk: especially with long‑term use.
  • Acute interstitial nephritis: kidney inflammation presenting with fever and flank pain.
  • Liver enzyme elevation: abnormal liver function tests.
