Indications & Clinical Uses

Ondansetron B. Braun is a selective 5‑HT3 receptor antagonist used to control nausea and vomiting associated with several therapeutic pathways.

  • Chemotherapy‑induced nausea and vomiting: reduces the frequency and severity of acute and delayed episodes.
  • Radiation‑induced nausea and vomiting: helps maintain patient comfort during and after radiotherapy.
  • Post‑operative nausea and vomiting: decreases the incidence of nausea and vomiting after surgical procedures.
  • Pregnancy‑related hyperemesis gravidarum (off‑label): may be considered when conventional anti‑emetics are ineffective.
  • Anticipatory nausea: used in patients who have developed a conditioned response to prior chemotherapy cycles.

How It Works

  • Ondansetron selectively blocks the serotonin 5‑HT3 receptors located on vagal afferent nerves in the gastrointestinal tract and in the chemoreceptor trigger zone of the brain. By inhibiting these receptors, it prevents serotonin‑mediated activation of the vomiting centre, thereby reducing the initiation of nausea and vomiting signals.

Side Effects

Side effects of ondansetron are generally mild but can vary in frequency and severity.

Common Side Effects

  • Headache: mild to moderate intensity.
  • Constipation: reduced bowel movements.
  • Diarrhoea: loose stools.
  • Fatigue: feeling of tiredness.
  • Dizziness: light‑headed sensation.
  • Injection site reactions: erythema or mild pain.

Serious or Rare Side Effects

  • Cardiac arrhythmia: QT‑interval prolongation, especially with rapid IV administration.
  • Severe allergic reaction: anaphylaxis, rash, or angio‑edema.
  • Serotonin syndrome: rare when combined with other serotonergic agents.
  • Hepatic dysfunction: elevated liver enzymes.
  • Vision changes: blurred vision or visual disturbances.
  • Severe hypotension: sudden drop in blood pressure.

Precautions & Warnings

When using Ondansetron B. Braun, clinicians should consider several safety measures to optimise patient outcomes.

  • Cardiac monitoring: assess baseline ECG and monitor QT interval in patients with cardiac risk factors.
  • Electrolyte balance: correct hypokalaemia, hypomagnesemia, or hypocalcaemia before administration.
  • Drug interactions: review concomitant medications that may prolong QT interval.
  • Pregnancy and lactation: weigh benefits against potential risks; use only if clearly needed.
  • Pediatric dosing: ensure dosing is calculated by a qualified healthcare professional.
  • Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake the vial.

Avoid Interactions

Ondansetron may interact with several drug classes, influencing its safety and efficacy profile.

Major Interactions (Avoid)

  • Class III anti‑arrhythmics (e.g., amiodarone, sotalol): may increase risk of QT prolongation.
  • Certain antipsychotics (e.g., haloperidol, ziprasidone): additive effect on QT interval.
  • Serotonergic agents (e.g., tramadol, linezolid): heightened risk of serotonin syndrome.

Moderate Interactions (Monitor Closely):

  • Macrolide antibiotics (e.g., erythromycin, azithromycin): may modestly prolong QT.
  • Fluoroquinolones (e.g., ciprofloxacin): similar QT‑related effect.
  • CYP 3‑ 2
