Indications & Clinical Uses

Ondansetron is a selective 5‑HT3 receptor antagonist used to manage nausea and vomiting in several clinical settings.

  • Chemotherapy‑induced nausea and vomiting: helps prevent and control episodes of nausea and vomiting caused by cytotoxic drugs.
  • Radiation‑induced nausea and vomiting: reduces the frequency and severity of nausea and vomiting after radiation therapy.
  • Post‑operative nausea and vomiting: decreases the incidence of nausea and vomiting following surgical procedures.
  • Hyperemesis gravidarum (severe pregnancy‑related nausea): used off‑label in some regions to control severe nausea when other treatments have failed.

How It Works

  • Ondansetron blocks serotonin (5‑HT3) receptors located on vagal afferent nerves in the gastrointestinal tract and in the chemoreceptor trigger zone of the brain. By inhibiting these receptors, it prevents the initiation of the vomiting reflex that is triggered by chemotherapeutic agents, radiation, or surgical stress, thereby reducing nausea and vomiting.

Side Effects

Common Side Effects

  • Headache: mild to moderate headache may occur.
  • Constipation: reduced bowel movements are reported.
  • Diarrhoea: occasional loose stools.
  • Fatigue: feeling of tiredness or reduced energy.
  • Dizziness: light‑headed sensations.
  • Injection site reactions (if applicable): mild redness or discomfort.

Serious or Rare Side Effects

  • Cardiac arrhythmia: QT‑interval prolongation may lead to irregular heart rhythms.
  • Severe allergic reaction: signs include rash, itching, swelling, severe dizziness, or trouble breathing.
  • Serotonin syndrome: rare but possible when combined with other serotonergic agents.
  • Hepatic dysfunction: elevated liver enzymes indicating liver stress.
  • Persistent visual disturbances: rare changes in vision.
  • Severe constipation or intestinal obstruction: rare gastrointestinal complications.

Precautions & Warnings

When using Ondansetron, consider the following precautions:

  • Cardiac monitoring: patients with known QT prolongation or electrolyte imbalance should be monitored.
  • Electrolyte balance: correct potassium, magnesium, and calcium levels before initiating therapy.
  • Drug interactions: review concomitant medications for potential serotonergic or QT‑prolonging effects.
  • Pregnancy and lactation: use only if clearly needed; discuss risks with a healthcare professional.
  • Renal or hepatic impairment: dose adjustment may be required; monitor organ function.
  • Store at below 25°C: keep in the original packaging, protect from moisture and direct sunlight.

Avoid Interactions

Ondansetron may interact with several medications, requiring careful review.

Major Interactions (Avoid)

  • Anti‑arrhythmic drugs (e.g., amiodarone, quinidine): may increase risk of QT prolongation.
  • Certain antibiotics (e.g., erythromycin, clarithromycin): can potentiate QT prolongation.
  • Serotonergic agents (e.g., tramadol, linezolid, SSRIs): heightened risk of serotonin syndrome.

Moderate Interactions (Monitor Closely)

  • Antifungal azoles (e.g., fluconazole, ketoconazole): may modestly increase ondansetron levels.
  • Antihistamines (e.g., diphenhydramine): may enhance sedative effects.
  • Oral contraceptives: potential reduction in contraceptive efficacy.
