Ondansetron is used to manage nausea and vomiting associated with several clinical pathways:

• Chemotherapy‑induced nausea and vomiting: reduces the frequency and severity of acute episodes.
• Radiation‑induced nausea and vomiting: helps control symptoms during and after radiotherapy.
• Post‑operative nausea and vomiting: decreases the incidence of nausea after surgical procedures.
• Pregnancy‑related nausea (off‑label): may be considered when other treatments are ineffective.
• Hyperemesis gravidarum (off‑label): used in severe cases under specialist supervision.

• Ondansetron blocks the action of serotonin at the 5‑HT3 receptors located in the chemoreceptor trigger zone of the brain and on vagal afferent nerves of the gastrointestinal tract. By preventing receptor activation, it interrupts the neural signaling pathway that initiates the vomiting reflex, thereby reducing nausea and vomiting.

Ondansetron is generally well tolerated, but adverse reactions can occur.

• Headache: mild to moderate intensity.
• Constipation: occasional bowel habit changes.
• Diarrhoea: transient loose stools.
• Fatigue: feeling of tiredness.
• Dizziness: light‑headed sensations.

Serious or Rare Side Effects

• Cardiac arrhythmia: QT‑interval prolongation, especially with IV use.
• Severe allergic reaction: rash, itching, swelling, or anaphylaxis.
• Serotonin syndrome: rare when combined with other serotonergic agents.
• Hepatic dysfunction: elevated liver enzymes.
• Vision changes: blurred vision or visual disturbances.
• Neuroleptic malignant‑like syndrome: extremely rare neurological emergency.

Before prescribing Ondansetron, consider the following safety points:

• Cardiac assessment: evaluate baseline ECG in patients with known heart disease.
• Electrolyte balance: correct hypokalaemia, hypomagnesemia, or hypocalcaemia before use.
• Drug interactions: review concomitant medications that may affect QT interval.
• Pregnancy and lactation: weigh benefits against potential risks; use only if clearly needed.
• Renal/hepatic impairment: adjust monitoring in patients with severe organ dysfunction.
• Store at below 25°C: keep in the original packaging, protect from moisture and direct sunlight.
