Indications & Clinical Uses

Opfolda (miguel miglustat) acts on the glycosphingolipid biosynthesis pathway and is prescribed for several rare disorders.

  • Type 1 Gaucher disease: improves clinical symptoms and reduces organ size when enzyme‑replacement therapy is insufficient.
  • Niemann‑Pick type C disease (NPC): slows disease progression and helps maintain neurological function in affected individuals.
  • Pompe disease (early onset) – used as adjunct therapy to enhance glycogen clearance in patients who cannot receive enzyme replacement.

How It Works

  • Miglustat inhibits the enzyme glucosylceramide synthase, reducing the synthesis of glucosylceramide and downstream glycosphingolipids. By limiting substrate accumulation, it helps restore normal cellular function in tissues where these lipids would otherwise build up, addressing the underlying metabolic defect in lysosomal storage disorders.

Side Effects

Miglustat may cause a range of adverse reactions, most of which are mild and manageable.

Common Side Effects

  • Diarrhea: watery, often mild to moderate.
  • Nausea: occasional upset stomach.
  • Weight loss: gradual reduction in body weight.
  • Flatulence: increased intestinal gas.
  • Abdominal pain: cramping or discomfort.

Serious or Rare Side Effects

  • Pancreatitis: severe abdominal pain with vomiting.
  • Severe hypersensitivity: rash, itching, swelling, or difficulty breathing.
  • Liver enzyme elevation: increased ALT/AST levels.
  • Peripheral neuropathy: tingling or numbness in extremities.
  • Mood changes: depression or anxiety.
  • Blood cell abnormalities: low white blood cells or platelets.
