Indications & Clinical Uses

Denosumab is a targeted therapy that interferes with bone‑resorbing pathways, providing benefit in several bone‑related conditions.

  • Osteoporosis (post‑menopausal): reduces vertebral, hip and non‑vertebral fracture risk.
  • Osteoporosis (male, high risk): lowers incidence of fractures in men with established bone loss.
  • Bone loss associated with glucocorticoid therapy: prevents loss of bone mineral density in patients receiving long‑term steroids.
  • Giant cell tumor of bone (recurrent or unresectable): delays disease progression and reduces need for surgery.
  • Hypercalcemia of malignancy (refractory): lowers serum calcium levels when other treatments have failed.

How It Works

  • Denosumab is a fully human monoclonal antibody that binds with high affinity to RANK ligand (RANKL), a key regulator of osteoclast formation, function and survival. By inhibiting RANKL, denosumab prevents osteoclast‑mediated bone resorption, leading to increased bone density and reduced fracture risk.

Side Effects

Adverse reactions to denosumab vary in frequency and severity.

Common Side Effects

  • Injection site reactions: redness, pain or swelling at the injection site.
  • Musculoskeletal pain: aches in joints, muscles or bones.
  • Upper respiratory tract infection: mild cold‑like symptoms.
  • Dermatologic reactions: rash or eczema‑like eruptions.
  • Hypocalcemia: low blood calcium levels, often asymptomatic.

Serious or Rare Side Effects

  • Osteonecrosis of the jaw (ONJ): exposed bone in the jaw that fails to heal.
  • Atypical femoral fractures: unusual breaks in the thigh bone.
  • Severe hypocalcemia: symptomatic low calcium requiring urgent treatment.
  • Severe hypersensitivity reactions: anaphylaxis or angioedema.
  • Infections requiring hospitalization: serious bacterial or fungal infections.

Precautions & Warnings

Denosumab should be used with careful clinical assessment and monitoring.

  • Calcium and vitamin D supplementation: ensure adequate intake to reduce hypocalcemia risk.
  • Renal impairment: assess kidney function before initiating therapy.
  • Prior radiation therapy to the jaw: evaluate risk of osteonecrosis of the jaw.
  • Concomitant anti‑resorptive therapy: avoid overlapping use with bisphosphonates.
  • Pregnancy and lactation: contraindicated; discuss alternatives with a specialist.
  • Store at 2-8°C: keep refrigerated, protect from light, do not freeze or shake.

Avoid Interactions

Denosumab may interact with several medicines, affecting safety or efficacy.

Major Interactions (Avoid)

  • Calcium‑containing antacids within 4 h of injection: may reduce absorption.
  • High‑dose vitamin D supplementation: can exacerbate hypercalcemia after discontinuation.

Moderate Interactions (Monitor Closely)

  • Bisphosphonates: overlapping anti‑resorptive effects increase risk of ONJ.
  • Immunosuppressants: may heighten infection risk.
  • Hormone replacement therapy: may alter calcium metabolism.
  • Anticoagulants: monitor for bleeding if severe hypocalcemia occurs.
  • Concomitant chemotherapy: assess infection risk regularly.
