Osmohale (Mannitol)

Osmohale is an osmotic diuretic that acts on renal pathways to promote diuresis and assist in fluid management.

• Acute kidney injury: helps facilitate urine output and reduce fluid overload.
• Chronic kidney disease: supports maintenance of fluid balance during disease progression.
• Pre‑operative preparation: used to reduce intravascular volume before certain surgical procedures.
• Hyper‑glucose management in diabetes (though it is off‑label but recognised in some guidelines).
• Treatment of intrarenal pressure increase during contrast enhanced imaging procedures to protect renal function and reduce risk of contrast‑induced nephropathy.

• Mannitol increases the osmolarity of the tubular fluid in the kidney, drawing water from the interstitial space into the lumen. This osmotic gradient reduces reabsorption of water and electrolytes, enhancing urine output. The resulting diuresis helps lower intravascular volume and decreases renal interstitial pressure, supporting renal perfusion and function during acute or chronic kidney stress.

Mannitol may produce a range of adverse reactions, from mild to serious, depending on patient factors and dosing.

Common Side Effects

• Headache 
• Nausea 
• Thirst 
• Transient rise in blood pressure 
• Electrolyte imbalance (e.g., hyponatremia)
• Mild cough due to inhalation formulation

• Pulmonary edema
