Ossica (Ibandronate Sodium Hydrate)

Ibandronate Sodium Hydrate is used to manage bone disease by targeting the skeletal remodeling pathway.

• Post‑menopausal osteoporosis: Improves the density of spine and hip bones and lowers the incidence of vertebral fractures.
• Male osteoporosis: Enhances bone strength and reduces fracture risk.
• Glucocorticoid‑induced osteoporosis: Helps to maintain bone mass when patients receive long‑term corticosteroid therapy.

• Ibandate as a nitrogen‑containing bisphosphonate binds to hydroxy‑apatite crystals in bone, where it is internalised by osteoclasts during resorption. The drug inhibits farnesyl‑pyrophosphate synthase, a key enzyme in the mevalonate pathway, disrupting prenylation of small G‑proteins and leading to osteoclast apoptosis, thereby reducing bone turnover.

Adverse reactions to ibandronate may vary in frequency and severity.

Common Side Effects

• Gastrointestinal discomfort: nausea, abdominal pain, dyspepsia.
• Musculoskeletal pain: transient bone, joint or muscle aches.
• Flu‑like symptoms: mild fever, chills.
• Headache.

Serious or Rare Side Effects

• Osteonecrosis of the jaw (ONJ).
• Atypical femoral fractures.
• Severe esophagitis or esophageal ulceration.
• Acute renal impairment.
• Hypocalcaemia.
• Severe hypersensitivity reactions.

Before the administration of Ossica, clinicians should evaluate patient risk factors and monitor relevant health parameters.

• Renal function: Assess the baseline creatinine clearance and adjust use if necessary.
• Calcium and vitamin D status: Ensure adequate supplementation to prevent hypocalcaemia.
• Esophageal disorders: Avoid in patients with active esophageal disease.
• Concomitant medications: Review the use of NSAIDs and corticosteroids.
• Store at below 25°C: keep in the original pack, protect from moisture.
